Coincontrol cleanup (e.g. add missing license)

• add missing license headers
• make compatible with Qt5
• enforce header cleanup style
• small code style cleanups
• rename Coin Control dialog into Coin Control Address Selection
• use default font for the windows labels (no monospace)
